The healthcare system is broken in many ways and very hard to navigate.

For Your Peace of Mind, LLC is aware that poor medical histories for the elderly and the lack of advocates often amount to poor medical outcomes.

  • We create a personal medical record in a printed or electronic format. This is kept up to date every time the client sees a medical professional.
  • Our  Patient Advocate is called to the ER whenever there is an emergency, provides the personal medical record and stays with the client as long as requested by the family. If family members are interested, they are taught how to advocate: the key questions to ask and their rights.
  • For continuity of care, our Patient Advocate meets with the healthcare team at the hospital the day before discharge and can prepare for any needed services in advance, so there is no gap in needed assistance.

Having a patient advocate on call 24/7, who has and can share the in-depth patient history with the Emergency Room staff and physician in charge can make the difference in providing the right care rapidly.

This same patient advocate can communicate with the patient and the family on the medical events and raise appropriate questions to clarify the situation.
