Family Facilitation

The elderly and their younger relatives can learn to work as a team as we hold the tough discussions about managing the future. These include: long-term care, crisis care, money, housing and legal documents (POA, Health Care Proxy, Living Will, standard Will, Trust, etc).

A trained facilitator and selected experts makes this a positive and productive process. Meetings are scheduled at your convenience.

elderly care professionalsWe identify the goals, hold pre-meetings to surface issues and suggestions and then orchestrate meetings to bring unity to the family and remove barriers to moving on.  Our family facilitations meeting agendas usually begin with the Client In-Depth Needs Analysis results. Other important family meeting topics are:

  • Patient Care and Advocacy Meeting
  • Money and Property Meeting
  • House Meeting
  • Legacy Meeting
  • Our facilitators provide education and reassurance to family members to know when the elder relative is declining and which services are needed for more support, e.g. companions, transportation, house cleaning, Hospice, etc.

    Our professionals provide navigation through the system to identify the possible responses for each new challenge and assist in defining thoughtful solutions for managing necessary changes with confidence.

    Additional reassurance can be provided by selecting unique services, such as:

     

    • Live streaming video visit between the elderly and their relatives using Skype technology, which saves travel costs and time.
    • Health and safety check-ins on the elderly on a weekly or bi-weekly basis, keeping tabs on any changes and reporting updates.

    We are also helpful to families in need of help with end of life issues and arrangements.
