- We create a personal medical record in a printed or electronic format. This is kept up to date every time the client sees a medical professional.
- Our
Patient Advocate is called to the ER whenever there is an emergency, provides the personal medical record and stays with the client as long as requested by the family. If family members are interested, they are taught how to advocate: the key questions to ask and their rights.
- For continuity of care, our Patient Advocate meets with the healthcare team at the hospital the day before discharge and can prepare for any needed services in advance, so there is no gap in needed assistance.
Having a patient advocate on call 24/7, who has and can share the in-depth patient history with the Emergency Room staff and physician in charge can make the difference in providing the right care rapidly.
This same patient advocate can communicate with the patient and the family on the medical events and raise appropriate questions to clarify the situation. |

The elderly and their younger relatives can learn to work as a team as we hold the tough discussions about managing the future. These include: long-term care, crisis care, money, housing and legal documents (POA, Health Care Proxy, Living Will, standard Will, Trust, etc).
A trained facilitator and selected experts makes this a positive and productive process. Meetings are scheduled at your convenience.
We identify the goals, hold pre-meetings to surface issues and suggestions and then orchestrate meetings to bring unity to the family and remove barriers to moving on. Our family facilitations meeting agendas usually begin with the Client In-Depth Needs Analysis results. Other important family meeting topics are:

Our facilitators provide education and reassurance to family members to know when the elder relative is declining and which services are needed for more support, e.g. companions, transportation, house cleaning, Hospice, etc.
Our professionals provide navigation through the system to identify the possible responses for each new challenge and assist in defining thoughtful solutions for managing necessary changes with confidence.
Additional reassurance can be provided by selecting unique services, such as:

streaming video visit between the elderly and their relatives using Skype technology, which saves travel costs and time.
- Health and safety check-ins on the elderly on a weekly or bi-weekly basis, keeping tabs on any changes and reporting updates.

Arranging for help with cleaning, repairs and safety adjustments at the elder's home is done through Preferred Vendors / Affiliates, who bid on and manage these efforts. The client sees the bids and decides on which bid to accept.
Considering and searching out safer living arrangements, such as assisted living, nursing home facilities and communicating with the family to make moving arrangements is a great help, especially when the family is no longer local to this area.
