About 3 hours is used to meet with you and your elderly relative(s) to scope out their medical, legal, accounting and social needs.

Input for this meeting includes:

• Identification: Date of birth, Social Security card, List of family members, addresses and phone numbers
• Health Information: Health insurance cards, Physicians' names, addresses, phone numbers, Medical history, Known allergies, Medications, details on recent hospital admissions and doctor visits, Functional abilities
• Financial information: Income/ assets summary: Bank information, Stocks, bonds, mutual funds, CD's, Owned property/land , Burial funds and or funeral contract
• Legal Information: Copy of the power of attorney, Copy of a trust that may include the financial assets
