For Your Peace of Mind, LLC will require the same of its employees and contractors. For Your Peace of Mind, LLC is not in the business of selling or trading  personally identifying information we collect about our clients to any third party without their permission.

There are circumstances, however, under which we allow access to our database by third parties, to the extent that those third parties provide us with services that help us operate the web site. For example, when you provide us with a credit card payment, the credit card processor and other entities for processing your payment will receive your information. From time to time, we may also engage the services of third parties to help us analyze use of the web site. These third parties are to use the personal information only in accordance with this policy.

There may also be occasions when we are legally required to provide access to our database in order to cooperate with investigations or other legal proceedings. In those instances, the information is provided only for that purpose.

 For Your Peace of Mind, LLC may collect personal information, including social security numbers, from our clients when necessary to perform services on your behalf. In compliance with Connecticut General Statutes Section 42-471, For Your Peace of Mind, LLC adheres to the following Privacy Protection Policy.

 As used in this policy, "personal information" means information capable of being associated with a particular individual through one or more identifiers, including but not limited to, a Social Security number, a driver's license number, a state identification number, an account number, a credit or debit card number, a passport number, an alien registration number or a health insurance identification number. As used in this policy, "personal information" does not include publicly available information that is lawfully made available to the general public from federal, state or local government records or widely distributed media.

 This Privacy Protection Policy describes the guidelines that For Your Peace of Mind, LLC follows for information obtained in connection with the services it renders. We will protect the confidentiality of personal information, limit access to such information and prohibit unlawful disclosure. We will not disclose your personal information, including your social security number except as authorized by you. Access to the social security numbers of employees or clients is limited to those with a business need to access such information in order to fulfill a necessary business function or as required by law.    

 For Your Peace of Mind, LLC will safeguard data, computer files and documents containing your "personal information" in at least the following ways:

  • Maintain written documentation containing personal information in a secure location with access limited to only those persons or entities necessary to execute their respective job functions
  • Provide awareness and/or training to contractors, employees and staff regarding the privacy protection policy
  • Ensure compliance with our Privacy Protection Policy through quality assurance/control
  • Maintain adequate virus/firewall protection on any computer that the company has in its possession

A Patient Advocate provides the exchange of information between the Patient & Health Care Provider in order to facilitate effective treatment and services.

Our For Your Peace of Mind Patient Advocates are all certified EMT-B, EMT-I, Paramedic, Medical Technicians or higher level medically certified professionals in the State of Connecticut.  They perform regular home visitation, home safety checks and provide detailed health records and advocate on behalf of the Client to Hospital Staff when a medical emergency occurs.  The Patient Advocates convey information to family as appropriate.  Patient Advocates do not act  within the scope of their certified practices while on duty with For Your Peace of Mind. However, their professional knowledge and rapport with the healthcare team is invaluable.

About 3 hours is used to meet with you and your elderly relative(s) to scope out their medical, legal, accounting and social needs.

Input for this meeting includes:

  • Identification: Date of birth, Social Security card, List of family members, addresses and phone numbers
  • Health Information: Health insurance cards, Physicians' names, addresses, phone numbers, Medical history, Known allergies, Medications, details on recent hospital admissions and doctor visits, Functional abilities
  • Financial information: Income/ assets summary: Bank information, Stocks, bonds, mutual funds, CD's, Owned property/land , Burial funds and or funeral contract
  • Legal Information: Copy of the power of attorney, Copy of a trust that may include the financial assets

A For Your Peace of Mind Care Manager is a professional who has knowledge and experience in working with older people and their families. Many older people are no longer able to manage their own medical needs, require assistance in obtaining home care or health care services, need assistance with daily living arrangements, or making plans for long-term, future care needs.

We assign a care manager and a back-up care manager to each client. We want to foster a strong relationship between the client and the care managers, and enrich the client's experience with a trusted and familiar caregiver.

Many Care Managers have training in gerontology, social work, nursing or counseling. They also have knowledge about the cost, quality and availability of services in the community. Some of our For Your Peace of Mind Patient Advocates are also Care Managers.

Our affiliated attorneys and accountants are also on call for you.

Our Care Managers can arrange for clients to be taken to medical appointments. We do not transport clients at this time.

The Patient Advocate attends these appointments and act as an advocate for the client as well as a liaison for the family members, many of whom live outside of the area. We list your questions and communicate on the client and family's behalf with physicians and their staff.

We can coordinate with a client's physician and area pharmacies, some of which prepare prescription med boxes that assist a client to be able to take their medication as prescribed. These pharmacies also serve our client's needs by providing a delivery service. A care manager can also personally  educate the client on how to properly use the medications box.

While we do not directly provide medicines, we can coordinate with home health personnel so that they can assist the client in proper medication management.

For Your Peace of Mind services are billed privately and generally, not covered by Medicare or supplemental health insurance. Some clients may have a long-term health policy which may cover some services. You can contact a care manager by calling the For Your Peace of Mind office, by contacting the assigned care manager personally, by e-mail, or web chat. We also offer streaming video chat with the office or with your loved ones. Choose whatever manner of communication works best for you.

Some client families prefer a regularly scheduled meeting time. These are arranged with everyone's schedule in mind.

We are experienced in moving clients from their homes to assisted living facilities. We make sure that there is a "buddy" assigned in the new facility to ease the transition. Our community resources are also available to assist in the process. Greater or less assistance can be provided, depending on your needs. Following the initial needs assessment, the care manger will speak with you and/or your elderly relative to outline a particular plan of care tailored for your relative's specific needs.  Fees and costs are discussed and a plan of care agreed upon. The File of Life indicates that For Your Peace of Mind is on the emergency call list. All emergency personnel know to take this file with the emergency patient to the hospital.

If your elderly relative resides in an assisted living facility, For Your Peace of Mind is contacted by the facility. If the client is taken by ambulance to a hospital, the FYPOM Patient Advocate will meet the client in the emergency room, coordinate with emergency room personnel on the client's behalf, ease the client's apprehension and communicate with the family members as to the status of the client.

If a client is living at home independently or with the assistance of a home health aide, the same procedure is followed.

A care manager has knowledge of the local providers and accessibility to specialized resources within the community. The community resources are reviewed with the family and the client after the In-depth Needs Assessment, or annual review. You may also ask about them at any time through our website, e-mail or by phone.

The care manager can assist with sign ups for events, bus rides and classes, depending on your elderly relative's health. Transportation and companion issues will be  solved through community resources or referral services.

However, our patient advocates and care managers work closely with several home health agencies to coordinate home health care for clients who require the services of an aide, nursing staff, etc. We will give you a choice of several agencies with good reputations in this area.

Client services are on a "fee for service" basis.  We do require a signed contract, or  a "Release for Information and Authorization for Services"  plus a retainer. The retainer amount is based upon the complexity of the desired services. The contract can be can be cancelled at any time with a refund of unused retainer monies.

No. We are private pay, but we do advise clients when a service may be covered by insurance, so that they are not charged for our private services unnecessarily. It is important to note that Title XIX coverage differs from state to state.
